We ask this question of people who are aged 16 years old and over.It’s up to you to decide your gender. Choose what you think best suits you. This question is voluntary, so you can leave it blank if you prefer.

If your gender is the same as the sex recorded on your birth certificate when you were born, tick “Yes”.
If the answer is “No”, fill in the term you prefer to identify your current gender.
